Coverage by state

"Check your plan" means Accountable may be in network or covered as an out-of-network benefit depending on your plan; the coverage check tells you which. Partner program counts are treatment providers and clinicians in that state that refer to or coordinate with Accountable. The Medicare offering is session-based and separate from the subscription; clinical services and billing for covered services come from the Insurance Offering Partners, and Medicare covers only when a licensed clinician finds a qualified clinical need after an evaluation visit.
