Accountable covers the year after treatment: the same certified peer recovery coach every week, at-home testing, a group every day, and care coordination with the member's clinicians and family. Delivered from home, in all 50 states, as a program the plan sponsors directly or in-network through our Insurance Offering Partners.
Treatment programs across the country already refer into Accountable.









Where we fit
ASAM is the American Society of Addiction Medicine. Its criteria are the standard clinicians and insurers use to decide what level of care someone needs. The newest edition includes Level 1.0, long-term remission monitoring: regular coaching sessions and check-ins after treatment, watching for early signs of trouble, and getting someone back into care quickly if they need it.
That is what Accountable does, delivered by certified peer coaches with at-home testing, so we describe our support as aligned to that level. We are not certified by ASAM, and we are not treatment. We sit after the discharge date and alongside outpatient care, therapy, and medication, for the period when a member is at home.
What a member gets
A certified peer recovery specialist who is in recovery themselves, on video, thirty minutes at a time, once, twice or three times a week depending on acuity.
A breathalyzer check twice a day with facial verification, and random saliva screens on recorded video. Devices ship to the member. Results go only to the people the member has chosen.
Group recovery meetings on video every day, led by a coach.
Their therapist, their prescriber, their treatment program, a parent, a partner. The member picks who is in the know and how much each person sees. A program or plan hears from us only with a signed release.
If a member misses a session or a check-in, or tests positive, nothing automatic happens. They get a message from their coach, not a report to anyone else, and nobody is dropped for a relapse. If we are worried about their safety, we tell them first and help them get to clinical care.
How we work with a plan
By form or phone. We reach the member within one business day, and a first session can happen as soon as the next business day. Tell us before discharge and a coach is ready the day they get home.
You see the claims, and with a data-sharing agreement and the member's signed release, your care team gets longitudinal reporting across the members we serve, plus a call from the coach when something changes.
Available as a sponsored program the plan pays for directly, or in-network through our Insurance Offering Partners, the medical groups that provide clinical services through Accountable and bill covered services under your plan's rules. Some members are covered through their health plan today, and Medicare may cover care in select states when a clinician finds a qualified clinical need.
Data sharing is opt-in and member-controlled under HIPAA and 42 CFR Part 2. Members choose exactly who sees what, and can change it any week.

Members leaving residential or IOP, in the months when return to use is most likely.
