If you're on naltrexone or the Sinclair Method

Naltrexone quiets the craving.
We're the person expecting you Thursday.

The pill takes the edge off. It can't notice when you skip a dose, and it can't pick up when the week goes sideways. A recovery coach who's been where you are does both, every week, alongside whoever writes your prescription.

What You Are Accountable is

A recovery coach who's been there,
plus the structure to make it stick.

You Are Accountable is virtual recovery support. You get a certified recovery coach who is in long-term recovery, a weekly one-on-one video session with that same coach, remote breathalyzer monitoring you control, and a Care Team you can reach between sessions. It works alongside your doctor, your therapist and your medication. We don't prescribe and we don't replace treatment.

Every week

Your coach, on video

The same person each week, 50 minutes, about your actual week. They've been through it and have a national peer credential.

At home

Remote breathalyzer

A small device that builds a record over time. You decide who sees the results: your coach, your prescriber, a family member, or only you.

Between sessions

Messaging and a Care Team

Message your coach, or call the Care Team 8am to 8pm ET. The hard day doesn't have to wait for Thursday.

If you want them

Optional groups

Daily peer groups you can join or skip, and a weekly group for family members. Nothing is required beyond your coach session.

Aligned to the ASAM Criteria Level 1.0, Long-Term Remission Monitoring. Fully virtual, nationwide.

What the medication does

Two ways to take it.
One thing neither does.

Naltrexone blocks the opioid receptors that make alcohol feel rewarding. The FDA approved it for alcohol dependence in 1994, and in the COMBINE trial people on naltrexone with medical management had more days without heavy drinking than people on placebo. It's generic, cheap, and not habit-forming.

Daily naltrexone

One 50 mg pill every day, usually with a goal of not drinking. This is how most prescribers and online services use it. It lowers craving in the background so the plan around it has room to work.

The Sinclair Method (TSM)

One pill about an hour before you drink, every time you drink, with no requirement to stop. Over months the brain stops pairing alcohol with reward. It only works if the pill is taken every single time.

Either way, the medication reduces the pull. It can't fill the hour after work, repair trust at home, give you someone to tell when the plan breaks, or show you whether it's working. That part is ours.

The difference

Everyone says coaching.
Here's what it means here.

The medication services below each include some kind of support. The left column is how they describe it on their own sites. The right column is ours.

Who you talk to
At the medication servicesA health coach by message (Oar), a therapist (Sunflower), a coach inside the medication program (Ria), or coaching as an optional add-on (Choose Your Horizon).
At You Are AccountableA certified recovery coach who is in long-term recovery, assigned to you, who stays your coach. Your prescriber or therapist stays yours too.
How you meet
At the medication servicesMessaging with replies within 24 hours and one weekly group (Oar). A weekly therapy session (Sunflower). Weekly coaching meetings in the app (Ria).
At You Are AccountableA scheduled one-on-one video session every week, messaging in between, a Care Team you can phone, and optional peer groups any day you want one.
What gets measured
At the medication servicesSelf-report and check-ins. Choose Your Horizon adds at-home liver testing in its program tier.
At You Are AccountableA remote breathalyzer that builds a record over weeks. You decide who sees it. Most members share it with their prescriber.
What happens on a bad week
At the medication servicesYou message, and someone replies.
At You Are AccountableYour coach already has the reading, already has Thursday on the calendar, and already knows your pattern. You don't have to start the conversation.
For the Sinclair Method

TSM works when the pill is taken every time.
That's an accountability problem.

The method fails most often in week three or four, when you feel better, or when a drink comes up unplanned and the pill stays in the drawer. Every unprotected drink re-teaches the brain the link the method is trying to erase. The second problem is time: extinction takes months, and almost nobody keeps an honest log that long on their own.

  1. Your coach works your planIf TSM is the plan, the weekly session is about the pill-every-time rule, the week ahead, and the moments it tends to slip. Your goal stays your goal.
  2. The record keeps itselfBreathalyzer readings over time are the graph the method promises you. For TSM it reads as a pattern, and the pattern is what your coach and prescriber look at together.
  3. Somewhere to go when it breaksYour coach by message, the Care Team by phone, and an optional peer group any day you want one. The hard day is the day the method needs you most.

A coach's story

"I tried to do recovery the way I did everything else: figure it out myself. Those were the hardest three years of my life. Staying sober and being happy was not something I could do alone."

Common questions
Do I need to get naltrexone from you?

No. We don't prescribe anything. Use your own doctor or any of the services above. With your consent we coordinate with whoever you choose.

I'm doing the Sinclair Method and still drinking. Will my coach tell me to stop?

No. Your goal is your goal. If your plan is TSM, your coach helps you follow TSM: the pill every time, the record, the honest look at the trend. If your goal changes, the plan changes with you.

What does monitoring look like if I'm not abstinent?

For members on naltrexone we use remote breathalyzer monitoring. For TSM it reads as a pattern over time rather than a pass or fail. You control who sees the results.

Isn't the coaching at Oar or Ria enough?

It might be. If a message thread and a weekly group keep you on track, keep them. Members come to us when they want a specific person who expects them every week and a record that doesn't rely on memory. You can have both; we work alongside them.

Is this treatment?

No. You Are Accountable is recovery support aligned to the ASAM Criteria Level 1.0, Long-Term Remission Monitoring. It sits alongside medical care. If you need detox, medication management or therapy, we help you find it and stay with you through it.

Does insurance cover it?

Many commercial health plans cover You Are Accountable, with coverage expanding monthly. The coverage check is the fastest way to know. Cash plans start at $375 a month.

Do I have to go to groups?

No. Groups are optional. Some members use them daily, some never do. The only standing commitment is your weekly session with your coach.

Can my family be involved?

Yes, if you want them to be. Families can join sessions when it helps, and there is a weekly family group for relatives of members.
